Forms for New Members
In order to get paid by Broward County Public Schools you must have an ACH Form (direct deposit) on file. By completing and submitting the forms below to the BCAA Office
JUST ONCE, they will cover for you ALL SPORTS, and all public school games officiated.
Features
These forms only apply to Broward PUBLIC SCHOOLS assignments. Private, and charter schools will continue to use their own various payment methods (Arbiter Pay, checks issued on site, checks mailed to your home etc.)
What is required
Each official must complete a Request for Taxpayer Identification - W-9 form
Each official must complete a new direct deposit authorization - ACH Payment Agreement
PRINTOUT - the TWO forms below and submit them with a cancelled check to Dawn Turner

Direct Deposit Info - Name of Bank, Branch/State, Routing number, Account number, Account type Checking or Savings
A voided check to verify your bank details if depositing into a checking account
